Here's a number that should scare you: 1 in 2 women over 50 will break a bone.
Hip. Spine. Wrist. Not minor fractures. Life-altering fractures. Mobility gone. Independence gone. Osteoporosis is associated with a 47% increased risk of mortality, according to a 2026 study in Menopause.
But here's what most women don't know: the damage starts long before 50.
It starts in your 40s, during perimenopause, when you still feel fine and have no idea what's happening inside your bones.
Here's why bone loss accelerates during perimenopause - the hormonal cascade that starts years before screening typically happens.
The metric that predicts all of this? Your T-score. A bone density test (DEXA scan) gives you one number that compares your bone density to a healthy 25-35-year-old. That's it. That's the entire test. You lie down for 10 minutes and learn whether you're going to break a bone at 60.
At 40, you feel fine. Your bones are normal.
By 45, they're declining. But you don't feel it.
By 50, you might finally get screened. That's when the damage becomes visible.
By then, you've lost half a decade to preventable bone loss.
The window isn't at 50. It's right now.


This is where most women get it wrong. They think bone is static, fixed, something that just gets worse with age.
It's not. Bone is living tissue that responds to stress. When you lift heavy things, your skeleton senses that demand and rebuilds itself stronger. This is called bone remodeling, and it happens in response to mechanical load.
The research is clear: women doing heavy resistance training 2-3 times per week for just 6-8 months increase lumbar spine bone density by 1-3%. Women doing nothing? They lost density over the same period.
One study tracked women aged 40-50 over six months of resistance training. The training group increased overhead press strength by 125%, leg press by 86%, and lumbar spine bone density went up 1%. The control group's declined by 0.36%.
Another study looked at postmenopausal women with osteopenia and osteoporosis. Eight months of heavy resistance exercises (deadlift, back squat, overhead press) at 80-85% max effort, twice weekly, plus jump squats produced measurable increases in bone density.
This isn't theoretical. This is measured. Quantified. Proven.


Not all strength training is equal.
Bone responds to heavy load. Compound movements that create mechanical stress on your skeleton work best.
Deadlifts. Back squats. Overhead press. Romanian deadlifts. Bent-over rows. These load your skeleton in different patterns. Your body adapts by getting stronger.
What doesn't work as well: light weights, high reps, machines, Pilates, yoga, walking. Pilates, yoga, and light weights have their place for balance and mobility, but for building bone density, heavy resistance training is in a different category. The data gap is massive.
The key is load and progression. You need to challenge your bones with increasing weight over time.

Two to three sessions per week. That's it.
Each session: deadlift, back squat, overhead press. 3 sets of 8-10 reps. Use a weight that feels challenging by rep 8-9, but you're not grinding or going to failure. Rest 2-3 minutes between sets. Forty minutes, done.
You don't need to lift heavy enough to terrify yourself. You need consistent, progressive resistance over time. Here's the heavy resistance training protocol that actually works for women over 40. Over 8 weeks, you'll get stronger. Over 6 months, your T-score moves.
Not dramatically. But measurably. And that matters.
You can test now at 40, see where you're starting from, and build bone while your body is still responsive to the stimulus. Or you can wait until 65, find out your T-score is in the danger zone, and try to fix it then with medication and exercise.
One is prevention. One is damage control.
